2020 Agency Application

All applicants are required to have or create an account to complete and submit your application.  Keep your user name and password handy so that you can "save a draft" and return to complete your submission.  Also this user name and password will continue to be used for agency reporting and future applications.

Incomplete applications will not be accepted.  Do not leave any blanks; enter N/A if not applicable.

This grant application is for agencies in Bonneville, Clark, Fremont, Jefferson, Lemhi, Madison, and Teton counties looking to receive program funding through United Way of Idaho Falls and Bonneville County. Please read the description that accompanies each question carefully before submitting your organization's information to ensure your understanding of eligibility and documentation required. Below are links to the budget worksheet template and the logic model template that are to be completed and uploaded as part of the application submission. You may save the electronic form and return to it at any time by creating or using a login and password. Please review each of the attached links and complete all information as listed in the instructions.

Agencies seeking funding for more than one project or program will be required to fill out an application for each.

Applications must be submitted by NOON, April 20, 2020.

Upon successful submission, a copy of the grant will be sent to the email addresses specified in the Executive Director and Program Contact lines. If you do not receive this confirmation, please contact the UWIF office for verification of successful submission.

USEFUL DOCUMENTS:

Please do not include any additional information such as flyers, brochures, handbooks, etc. You will have the opportunity to provide this information as part of your presentation. Please note, agencies selected to receive funding will be required to provide a certificate of liability and worker's compensation insurance as well as a completed W-9 from the IRS.
